If a first time guest or over a year since you visited Bliss, you will need to complete a Client Intake Form prior to your massage. The intake form is available to the right should you wish to save time and complete it in advance of your session.  If you are unable to complete the intake form ahead of time, please build in an extra 5-10 minutes to your arrival time to allow for completion in-office. This process allows the therapist to get to know your goals in seeking out massage, as well as to customize your session to your specifications and preferences.

The Client Intake Form includes operating policies which all guests of Bliss Massage Therapy must indicate understanding and adherence to by initialing and signing, prior to receiving any massage therapy services from this establishment.  

Please be aware that I utilize essential oils by infusing steamed towels. If you have allergies, sensitivities or object to their use for your session, please let this be known so that  your session may be customized accordingly.
